Nasdaq’s public short-interest page for BCYC currently lists the data as unavailable, so this should be treated as directional rather than a real-time official tape read.

Importantly, there is no obvious Reg SHO distress signal in the publicly available framework. Reg SHO threshold status generally requires aggregate fails to deliver of at least 10,000 shares and at least 0.5% of shares outstanding for five consecutive settlement days, and NasdaqTrader notes that Nasdaq market threshold lists remain displayed through NasdaqTrader.

WHAT THIS MEANS FOR YOU

Looking at the analysts results and price coverage, those who cover the company rate as a MODERATE BUY with 5/8 buy ratings. Keep in mind analyst ratings ALWAYS must be taken with a grain of salt as bearish analysts do not have long careers for obvious reasons.

The stock is currently trading at $4.05. The lowest analyst estimate has this one trading at $5.00. The upside scenario is at $36 ~9x upside with downside potential over the longer timeframe.

If we look at options pricing, and specifically IMPLIED VOLATILITY we can get a sense of the size of the move the traders are projecting. IF you do the math you get about a ~+140% move to the upside, and a -100% move to the downside. That would put the options projected price projections at ~$9.60 to the upside and ~$0.00 to the downside. Again, this is for a play on the short term catalyst which is a different timeframe from the 1yr analyst targets shown above.

May layoffs hit nearly 7,000, with most cuts from Takeda, BioNTech

In May 2026, biopharma companies announced or enacted layoffs affecting 6,956 employees, a 47% increase year-over-year and nearly double February’s monthly high, according to BioSpace. The bulk of cuts were disclosed by BioNTech and Takeda, which together accounted for 6,360 planned reductions. BioNTech will close several manufacturing facilities by 2027, impacting around 1,860 positions, including previously reported layoffs in Singapore. Takeda’s restructuring, detailed in its fiscal year 2026 earnings presentation, involves approximately 4,500 job cuts, with at least 634 in the U.S. confirmed via WARN notices. Additional workforce reductions were reported by AbbVie, Amicus Therapeutics, Johnson & Johnson, Novartis, and Valneva.

Merck, Johnson & Johnson, Novartis, AbbVie, Takeda, Valneva, and Amicus Therapeutics (post-BioMarin acquisition) have all announced layoffs ranging from several dozen to several thousand roles globally, often citing strategic realignment, site closures, and the need to redirect resources as key motives. These industry-wide measures signal ongoing adjustments to market demands, pipeline challenges, and the pursuit of financial efficiencies.

Genentech R&D layoffs hit top exec who’s been with the biotech nearly 30 years

Genentech has implemented restructuring within its research and early development group (gRED), resulting in targeted layoffs and the departure of key executives, including Vishva Dixit, who had served since 1997 and was most recently vice president and senior fellow, physiological chemistry, research biology. The reorganization is aimed at aligning the Roche subsidiary’s investments and capabilities with its core therapeutic focus areas to advance its pipeline and accelerate the delivery of new medicines. The company has closed its infectious disease and physiological chemistry units and downsized teams in early clinical development, development sciences, and translational medicine, but the exact number of affected employees has not been disclosed.

Other high-level departures include Todd McDevitt, vice president and head of cell therapy, and Man-Wah Tan, vice president and senior fellow of infectious diseases. Genentech acknowledged Dixit’s significant scientific contributions, particularly in the understanding of cell death, immunity, and inflammation, which have influenced ongoing drug discovery efforts. Despite the layoffs, Genentech stated that it continues to hire within gRED in the U.S., emphasizing its commitment to evolving its workforce to maintain scientific leadership and deliver impactful therapies.

Novartis’ $12B Avidity buy pays dividends with Phase 1/2 muscular dystrophy win

Novartis’ $12 billion acquisition of Avidity Biosciences is showing early clinical promise, as demonstrated by positive interim results from the Phase 1/2 FORTITUDE trial evaluating delpacibart braxlosiran (del-brax) in patients with facioscapulohumeral muscular dystrophy (FSHD). In this study of 90 patients, del-brax met primary and key secondary biomarker endpoints by significantly reducing plasma levels of KHDC1L, a disease marker, as well as lowering creatine kinase concentrations, suggesting both strong target engagement and reduced muscle damage. While detailed data were not released, previous FORTITUDE results indicated functional improvement in treated patients on walk-run and muscle function tests over 12 months, in contrast to a decline in placebo recipients.

Strategically, these results bolster Novartis’ investment in RNA-based therapies for rare neuromuscular diseases. The company is progressing del-brax into the Phase 3 FORTITUDE-3 trial in FSHD, aiming to enroll 200 patients to further assess muscle performance outcomes, but has not yet disclosed regulatory filing plans. FSHD is a progressive genetic disorder affecting up to 87,000 people in the US and Europe, caused by toxic expression of the DUX4 protein, which del-brax is designed to suppress. The Avidity acquisition also brought Novartis additional RNA therapies in development for Duchenne muscular dystrophy and myotonic dystrophy, as well as a portfolio of preclinical neuromuscular programs, enhancing the company’s pipeline in this specialty market.

Update: Parabilis raises industry-record $670M IPO

Parabilis, formerly known as FogPharma, has set a new record for the largest biotech IPO by raising $670 million, surpassing both its original $476 million target and the previous record set by Kailera Therapeutics. The upsized offering involved the sale of 33.5 million shares of common stock at $20 per share, with underwriters receiving a 30-day option to purchase an additional 5 million shares at the same price. The company’s Nasdaq debut also indicates strong investor confidence amid a warming IPO environment for biotech firms.

The capital raised will primarily fund the advancement of Parabilis’ lead asset, zolucatetide, a peptide derived from its Helicon platform, through dose-expansion and into Phase 3 trials for desmoid tumors, as well as other indications such as familial adenomatous polyposis and hepatocellular carcinoma. Parabilis recently secured a partnership with Regeneron Pharmaceuticals, which includes a $75 million investment and potential milestone payments up to $2.3 billion, focusing on the development of antibody-helicon conjugates (AHCs). The company trades under the ticker PBLS and joins nine other biotechs that have gone public in the U.S. in 2024, benefiting from robust investor demand despite general unpredictability in biotech IPO pricing.

Parabilis sets a record with a $670M biotech IPO

Parabilis Medicines, a biotech company focused on developing therapeutics for “undruggable” targets, raised $670 million in its initial public offering, setting a record for the largest-ever IPO by a venture-backed biotechnology company. The company sold 33.5 million shares at $20 each and additionally secured $75 million through a private placement to its new research partner, Regeneron Pharmaceuticals. Parabilis, led by former Johnson & Johnson executive Mathai Mammen and founded by Gregory Verdine, had previously raised over $800 million in private funding. Its proprietary “Helicons” platform develops helix-shaped peptide drugs targeting intracellular cancer-driving proteins. The most advanced program, zolucatetide, is being developed for the beta-catenin pathway implicated in cancer and has shown early promise in desmoid tumors; a Phase 3 clinical trial in that indication is planned for the first half of 2027, with further studies ongoing for other cancer types and an inherited condition linked to polyp growth.

This IPO extends a wave of large biotech stock offerings in 2026, with 12 companies raising over $4.1 billion collectively and a median IPO haul more than double last year’s figures. Parabilis’ offering surpasses previous industry records, including those set by Moderna in 2018 and Kailera Therapeutics earlier in 2026. The sustained uptick in biotech IPO sizes reflects heightened investor interest and favorable capital markets, occurring alongside major technology IPOs from companies such as OpenAI and SpaceX. Parabilis continues preclinical work on additional pipeline assets in areas such as prostate cancer, positioning itself as a leading player among next-generation oncology therapeutics. The company began trading under the ticker symbol “PBLS.”

Takeda’s AI-designed pill bests BMS’ Sotyktu in head-to-head psoriasis trial

Takeda’s investigational oral psoriasis drug, zasocitinib, demonstrated significantly greater efficacy than Bristol Myers Squibb’s Sotyktu in a Phase 3 head-to-head trial, achieving complete skin clearance in over 35% of patients at 16 weeks—more than 2.5 times Sotyktu’s response rate. Zasocitinib also outperformed Sotyktu on secondary endpoints, including PASI 90, indicating at least 90% improvement in skin lesions, and showed separation of efficacy as early as week 8. Additional late-stage studies supported these results, with total skin clearance rates between 25.2% and 33.4% at 16 weeks, outperforming both placebo and Amgen’s Otezla.

Takeda acquired zasocitinib from Nimbus Therapeutics for $4 billion in December, underscoring its strategic interest in the asset, which was developed using artificial intelligence and selectively targets TYK2 with 24-hour IL-23 inhibition. The drug’s strong Phase 3 results are expected to “de-risk regulatory approval,” according to industry analysts, with the oral psoriasis market projected to exceed $5 billion by 2030. Takeda plans to file for regulatory approval with the FDA and other international agencies during the current fiscal year, positioning zasocitinib as a potential leader in the oral psoriasis treatment landscape.

Takeda’s $4B TYK2 drug tops Bristol Myers’ Sotyktu in head-to-head test

Takeda announced that its experimental TYK2 inhibitor, zasocitinib, demonstrated statistical superiority over Bristol Myers Squibb’s Sotyktu in a head-to-head trial for moderate-to-severe plaque psoriasis. While detailed data were not provided, Takeda reported that, after 16 weeks, over one third of patients receiving zasocitinib achieved PASI 100 (complete skin clearance), compared to less than 14% in historic Sotyktu trials—more than doubling Sotyktu’s effectiveness. Zasocitinib also outperformed Sotyktu across all secondary endpoints, including measures of partial lesion clearance. This marks the second instance where zasocitinib has beaten an approved therapy in a head-to-head trial, following earlier positive results against Amgen’s Otezla.

Following its $4 billion acquisition of zasocitinib from Nimbus Therapeutics, Takeda is positioning the drug as a leading oral option in a fast-growing, yet competitive, autoimmune market. However, the company faces challenges from both existing oral and injectable psoriasis treatments, such as J&J’s Tremfya and AbbVie’s Skyrizi, which have achieved higher complete clearance rates in prior studies (though not directly compared). Bristol Myers recently de-emphasized dermatology promotion for Sotyktu amid lower-than-expected commercial performance, with 2025 revenues of $291 million. Looking forward, Takeda aims to capitalize on growing physician and patient interest in oral therapies and anticipates key phase readouts for zasocitinib in inflammatory bowel disease later in 2024, as prior attempts by TYK2 inhibitors, including Sotyktu, failed in that indication.

Novo hit with cybersecurity breach, urges vigilance among trial participants

Novo Nordisk disclosed a cybersecurity breach involving unauthorized access to personal data of clinical trial participants. According to the company, only a limited amount of anonymized information was affected, such as sex, year of birth, biomarkers, lifestyle factors, and randomized patient IDs. Novo assured that no data was directly linked to individual names or identifiers and that the breach should not enable third parties to identify participants. The company urged trial participants to remain vigilant and report any suspicious activity and has initiated an investigation with cybersecurity experts, taking interim steps such as deactivating certain internal systems, though it did not specify which systems or the potential impact on patient experience.

The timing of the breach comes amid strong commercial momentum for Novo Nordisk. While previously perceived as having ceded leadership in the GLP-1 market to Eli Lilly—whose tirzepatide products generated $36.51 billion in revenue last year—Novo’s semaglutide franchise (Ozempic, Wegovy, Rybelsus) closely followed. Novo recently achieved a notable milestone with FDA approval of the oral form of Wegovy, reaching one million patients within 16 weeks and generating $355 million in Q1 sales. Despite competition from Lilly’s Foundayo, Novo’s oral obesity drug has been described by management as off to a “record-breaking start” in the U.S., reflecting resilience and growth in the face of competitive and operational challenges.

Lilly bets up to $1B+ with AlzeCure in search for next Alzheimer’s act

Eli Lilly has entered into a back-loaded licensing agreement with Swedish biotech AlzeCure Pharma, centered on ACD680, a small-molecule gamma-secretase modulator targeting amyloid-beta pathology in Alzheimer’s disease. The deal includes $10 million upfront and unspecified development and commercial milestone payments, with a potential total value exceeding $1 billion. AlzeCure is eligible to receive tiered mid-single-digit royalties on future sales. ACD680 aims to reduce the production of Aβ42, a pathogenic amyloid-beta variant implicated in characteristic Alzheimer’s plaques, while increasing production of less harmful amyloid-beta forms, which may position it both as a therapeutic and preventive option for Alzheimer’s, as stated by the company’s CSO and CEO.

Strategically, this alliance expands Lilly’s Alzheimer’s portfolio beyond its recently FDA-approved anti-amyloid antibody Kisunla, which was cleared in July 2024, trailing Biogen and Eisai’s Leqembi that achieved full FDA approval in July 2023. BIOTECH PUBLIC MARKET UPDATE

For the week, the public indexes were UP, with the S&P +0.5% & DOW moving +0.7%. For the year the public indexed are UP the S&P is up +7.7% and DOW is up +5.5%. The XBI (the biotech index) comes in UP +1.77% for the week and is up +9.9% for the year.

Macro Update

AI's Power Hunger Is Becoming A Biotech Problem

Most investors view the AI infrastructure buildout as a technology story. Nvidia sells chips. Hyperscalers build datacenters. Utilities scramble to keep up. Biotech appears completely disconnected from this trend.

Look closer. Modern biotech increasingly runs on the same infrastructure AI is consuming. Cell therapy manufacturing, biologics production, sequencing centers, high-performance computing clusters, cryogenic storage facilities, and AI-enabled drug discovery platforms all require significant amounts of power. The assumption has always been that electricity would remain abundant and relatively cheap. That assumption is starting to break down.

The scale of the AI buildout is difficult to appreciate. Utilities across major markets are receiving power requests that would have seemed absurd only a few years ago. New datacenters are demanding hundreds of megawatts at a time. In some regions, utility interconnection queues stretch years into the future. Every megawatt directed toward AI infrastructure is a megawatt unavailable for other industrial users. The result is rising power costs, longer project timelines, and growing competition for electrical capacity.

This matters for biotech because many of the industry’s most promising areas are also among its most power-intensive. Cell therapy manufacturing facilities require highly controlled environments operating around the clock. Advanced biologics manufacturing relies on increasingly complex production infrastructure. AI drug discovery companies are now competing for the same computational resources as technology firms. Investors spend enormous amounts of time debating clinical endpoints, regulatory outcomes, and competitive landscapes. Few consider that one of the next bottlenecks for biotech scale-up may simply be access to affordable power.

The market still views energy constraints as an AI problem. That is likely a mistake. Over the next decade, electricity becomes a strategic input for biotechnology in the same way semiconductor capacity became a strategic input for artificial intelligence. The companies best positioned to scale may not be the ones with the best science. They may be the ones that secure infrastructure before everyone else realizes it has become scarce.

Introduction

This week we’re stepping back from individual clinical trials and asking a bigger question. What happens when the factory disappears?

For decades, cell therapy has been defined by complexity. Collect cells. Ship them. Engineer them. Expand them. Ship them back. The process works, but it was never designed to scale. A new generation of companies is attempting something far more ambitious. Instead of manufacturing cell therapies in specialized facilities, they want to manufacture them directly inside the patient. If successful, in vivo CAR-T has the potential to reshape not only oncology but the broader future of immune engineering.

We’ll also take a brief look at a seemingly unrelated trend unfolding outside biotech. The race to build AI infrastructure is consuming enormous amounts of power, creating a new class of bottlenecks that few investors are paying attention to. As biotechnology becomes increasingly dependent on advanced manufacturing, computation, and data-intensive research, access to energy is becoming a strategic resource. The next constraint on innovation may not come from biology or regulation. It may come from the electrical grid.

As always, the goal is to identify where the consensus narrative breaks down and where the next opportunity is forming before the market fully appreciates it.

Why in vivo CAR-T is the next real platform fight

CAR-T has always had a manufacturing problem pretending to be a biology problem. We pull T cells out of a patient, ship them, engineer them, expand them, test them, freeze them, ship them back, condition the patient, then reinfuse a living drug weeks later. It works. It also looks insane when you explain it to anyone outside cell therapy.

In vivo CAR-T says the quiet part out loud. Stop manufacturing the cell therapy in a clean room. Manufacture it inside the patient. One IV dose. The vector finds T cells. The T cells express a CAR. The body becomes the bioreactor.

That is why Lilly paid up to $7 billion for Kelonia in April 2026, including $3.25 billion upfront, and why AbbVie bought Capstan for $2.1 billion in 2025. These are not normal early-stage cell therapy deals. These are platform land grabs. Lilly is buying oncology optionality. AbbVie is buying autoimmune optionality. Kite bought Interius for $350 million because the incumbent CAR-T players cannot ignore a technology that attacks their own cost structure.

The bull case is obvious. The bear case is better.

The bull case says in vivo CAR-T removes leukapheresis, bespoke manufacturing, vein-to-vein delays, and maybe lymphodepletion. That changes access. It changes gross margins. It lets CAR-T move from late-line blood cancer into autoimmune disease, community oncology, and eventually repeatable immune reset medicine.

The bear case says you are injecting a genetic payload into a sick person and hoping it finds the right immune cells, at the right dose, for the right duration, without creating a runaway immune event. That is not a small ask. Ex vivo CAR-T looks clunky because every variable gets controlled outside the body. In vivo CAR-T looks elegant because many of those variables disappear from view. They do not disappear from biology.

The core question is not whether in vivo CAR-T can make CAR-positive T cells. It can. The better question is whether it can make the right number of useful CAR-T cells inside real patients with bad immune systems. Cancer patients have exhausted T cells. Autoimmune patients often have chronic inflammation and prior immunosuppression. The preclinical monkey does not tell you how a fifth-line myeloma patient responds.

The field is already splitting into multiple technology stacks

One of the biggest mistakes investors make is treating in vivo CAR-T as a single category. It isn’t.

The term describes an outcome, not a technology. Every company wants to engineer immune cells inside the patient. The methods differ dramatically. Those differences will determine where each platform wins and where it fails.

Some companies use targeted lipid nanoparticles carrying mRNA. Others use engineered viral vectors. A third group is pursuing polymeric and other non-viral delivery systems. Each architecture makes different tradeoffs around persistence, safety, manufacturability, and redosing.

The split is already becoming visible.

mRNA platforms such as Capstan are optimized for transient immune programming. That makes them attractive in autoimmune disease, where physicians want a deep reset without permanent immune modification.

Viral platforms such as Kelonia are pursuing durable expression and sustained activity. That profile fits oncology, where persistence often drives efficacy.

Polymeric and other non-viral systems sit somewhere in between. They receive less attention today, but they may ultimately offer the best balance between efficacy, safety, and scalability.

Most investors focus on the payload. They ask whether a company targets CD19 or BCMA. The more important question is whether the delivery system can reliably generate a therapeutic response across diverse patients. Antigens change. Delivery platforms endure.

That is why AbbVie bought Capstan and Lilly bought Kelonia. Neither company was buying a single asset. They were buying competing visions of how immune cells will be programmed inside the body over the next decade.

The market still talks about in vivo CAR-T as if one platform will dominate every indication. The data suggest the opposite. Oncology and autoimmune disease have different requirements. The eventual winners may emerge from entirely different technology stacks.

The hidden theme is control

Most people talk about access and cost. That is fine. It is also the obvious take.

The real bottleneck is control.

Ex vivo CAR-T gives you control over cell selection, transduction efficiency, expansion, release testing, dose, and product identity. In vivo CAR-T trades that control for speed. You no longer dose cells. You dose instructions. That changes the regulatory problem and the clinical problem.

A 70 kg patient with robust circulating T cells does not equal a 70 kg patient with lymphopenia, recent bispecific exposure, exhausted marrow, and steroid use. Same vector dose. Different cellular factory. Different product. The patient is not just receiving the drug. The patient is part of the manufacturing process.

That is the key investor insight. In vivo CAR-T will not be valued only by target antigen. It will be valued by how well the platform normalizes patient-to-patient variability.

Why autoimmune disease is the cleaner first market

Autoimmune disease is where this story gets interesting.

CD19 CAR-T has produced striking resets in autoimmune disease using ex vivo products. The biology is not subtle. Deplete pathogenic B cells hard enough and some patients look like their immune system reboots. That opens lupus, myositis, systemic sclerosis, myasthenia gravis, pemphigus, and other B cell driven diseases.

But ex vivo CAR-T is too heavy for most autoimmune markets. Patients are not terminal oncology patients. They care about infertility risk, infection risk, ICU risk, and long-term safety. Physicians will not casually send stable autoimmune patients through lymphodepletion and bespoke cell therapy logistics.

That is why Capstan mattered. Transient CAR expression solves a real autoimmune problem. You want a hammer, but not a permanent hammer. You want B cell depletion deeper than rituximab, with less infrastructure than ex vivo CAR-T. mRNA delivery gives you that design logic. It also creates a redosing question. If one pulse does not reset immunity, do you dose again, and what happens after the immune system sees the delivery vehicle twice?

Oncology is harder than the press release says

Oncology investors love in vivo CAR-T because it sounds like the natural successor to autologous CAR-T. That view skips over ugly biology.

Blood cancers grow while patients wait. So speed helps. But late-line cancer patients also have damaged immune systems. Their T cells are often exhausted, numerically weak, or recently hit by therapies that alter the same compartments you need for in vivo engineering. You can inject a perfect vector into a poor T cell pool and get a mediocre living drug.

Kelonia’s lead KLN-1010 targets BCMA in relapsed or refractory multiple myeloma and is already in Phase 1. That target choice is smart and dangerous. Smart because BCMA is validated. Dangerous because BCMA is crowded, expectations are high, and comparators exist. Investors will not give much credit for “we made CAR-T cells in vivo” if response depth, durability, CRS, ICANS, and infection burden look ordinary.

The first oncology wins probably come where speed beats perfection. A product that treats faster, reaches more patients, and avoids manufacturing failure can win even with somewhat weaker efficacy. But it cannot be weak. This field dies quickly if in vivo CAR-T becomes “convenient but less potent.”

The most interesting idea is not CAR-T

Here is the part people underweight.

In vivo CAR-T is a gateway drug for in vivo immune programming.

Once you can target T cells in the body, the CAR is just the first payload. You can deliver TCRs, cytokine circuits, exhaustion resistance modules, transient gene editors, tolerogenic programs, or suicide switches. You can also target NK cells, Tregs, monocytes, or tissue resident immune cells.

That means the platform companies are not just building cheaper CAR-T. They are building immune cell routers. The company that learns how to deliver genetic instructions to selected immune cells in vivo owns a much broader design space than CD19 or BCMA.

This is why the delivery layer matters more than the first asset. A mediocre first CAR can still prove targeting, biodistribution, expression kinetics, and redosing. A clean delivery dataset is worth more than one antigen program. Watch the biodistribution tables. Watch off-target transduction. Watch whether they show single-cell data. That is where the platform truth hides.

The short thesis

The short thesis is simple.

These companies are being priced as if delivery specificity is solved. It is not solved. It is improving.

The body does not like nanoparticles floating around. The liver catches things. The spleen catches things. Myeloid cells eat things. Antibodies bind things. Complement activates. Viral vectors bring insertion and immunogenicity questions. LNPs bring repeat dosing and innate immune activation questions. Polymer systems bring their own tolerability and CMC questions.

The FDA will ask uncomfortable questions. What exact cells were modified? Where else did the payload go? How long did expression last? What happens if a patient has low T cell counts? Can you rescue severe toxicity if the factory is inside the patient? What is the release test when the final product forms after dosing?

That last question is brutal. For ex vivo CAR-T, the product gets tested before infusion. For in vivo CAR-T, the final pharmacologic product emerges inside the patient. Regulators will not ignore that.

What nobody is saying out loud

In vivo CAR-T also changes power inside hospitals.

Today, major academic centers control much of cell therapy because they have the infrastructure. They have leukapheresis workflows, cell therapy units, ICU familiarity, and trained teams. If in vivo CAR-T becomes a true IV medicine, the center of gravity moves.

Community oncology wants this. Rheumatologists want this even more, assuming safety improves. Big Pharma wants it most. A scalable IV product fits their commercial machine. Bespoke cell therapy does not.

That is the second-order trade. In vivo CAR-T threatens not only autologous manufacturing. It threatens the institutional moat around CAR-T delivery. The winners become companies with delivery IP, scalable CMC, and disease-area sales infrastructure. The losers are not only weak cell therapy biotechs. The losers include service layers built around complexity.

The future probably looks split

Oncology and autoimmune disease will not converge into one product model.

Oncology needs potency. It needs expansion. It needs persistence. It probably accepts more risk. Viral in vivo platforms make sense there, especially if they show clean T cell selectivity and strong expansion in patients.

Autoimmune disease needs controllability. It needs repeatability. It needs safety in patients who have treatment options. Transient mRNA platforms make more sense there. The winning autoimmune product does not need to look like axi-cel. It needs to look like a deep immune reset that a rheumatologist can defend.

That split matters. Do not evaluate every in vivo CAR-T platform with the same lens. A transient CD19 mRNA product can look underpowered for lymphoma and still look perfect for lupus. A durable lentiviral BCMA product can look too risky for broad autoimmune use and still make sense in myeloma.

What to watch next

Watch these four things.

Target cell purity. If a company cannot show convincing T cell selective delivery in primates and humans, discount the platform.

Expression kinetics. Transient is not bad. Permanent is not good. The right answer depends on disease.

Patient variability. The key clinical dataset is not the average CAR-positive T cell percentage. It is the spread across patients.

Redosing. This is the sleeper issue. If the platform cannot redose cleanly, chronic and autoimmune markets shrink.

The investment setup

The field has entered the expensive proof stage.

Lilly, AbbVie, and Kite have already told you the strategic value. Lilly’s Kelonia deal is the loudest signal because the upfront alone was $3.25 billion. AbbVie’s Capstan deal confirms autoimmune demand. Kite’s Interius deal confirms incumbents are hedging their own model.

But big checks do not validate biology. They validate fear of missing the platform. Pharma buys these assets because the upside is existential. If in vivo immune engineering works, ex vivo cell therapy becomes the expensive first draft.

The best near-term public market angle is not “buy every in vivo CAR-T story.” That is how you get wrecked. The better angle is to map who owns delivery, who owns validated antigen biology, who owns autoimmune commercial infrastructure, and who owns toxicity management. The value will not distribute evenly.

Final read

In vivo CAR-T is one of the few biotech themes that deserves the hype and the skepticism.

The opportunity is real. A single infusion that programs immune cells inside the body changes cost, access, and disease scope. It turns CAR-T from a hospital-manufactured rescue therapy into a programmable drug class.

The risk is also real. The body is a messy factory. You do not get to QC the final product before it exists. You do not get clean patients. You do not get perfect delivery. You get biology.

That is why this field is compelling. Not because it makes CAR-T cheaper. Because it forces a bigger question.

Can we turn immune cells into programmable therapeutics without touching them outside the body?

If the answer is yes, CAR-T is only the first chapter. If the answer is no, the next few years will be a very expensive reminder that manufacturing complexity sometimes exists for a reason.

CONCLUSION

Today, the most interesting opportunities in biotech are emerging at the intersection of disciplines. The future of cell therapy is increasingly becoming a delivery problem. The future of drug discovery is increasingly becoming a computation problem. And the future of scaling biotechnology may increasingly become an infrastructure problem. Investors who focus only on clinical data risk missing the larger forces shaping the industry. The next generation of winners will not simply have the best biology. They will have the platforms, manufacturing capabilities, and infrastructure needed to translate that biology into products at scale. What Biotech Investors Need to Know

Anchor expectations to the VYD2311 development program rather than the existing PEMGARDA business. The central catalyst is the ongoing REVOLUTION program, particularly the DECLARATION Phase 3 study, which is designed to determine whether VYD2311 can demonstrate meaningful protection against symptomatic COVID in a broad population. The key question is whether the company’s engineered antibody approach can establish a durable, scalable alternative to traditional vaccination and support eventual regulatory approval for a substantially larger market than PEMGARDA currently serves.

Bulls will focus on the potential for VYD2311 to create an entirely new category of COVID prevention. Supporters point to the company’s antibody engineering platform, activity against conserved viral epitopes, durability profile, growing commercial infrastructure, and the opportunity to address a large population of vaccine-hesitant, vaccine-averse, or under-vaccinated individuals. Positive efficacy and safety data could validate the broader platform, support expansion beyond immunocompromised patients, and drive a re-rating from a niche COVID company toward a commercial infectious disease franchise.

Bears will focus on the challenges that have historically plagued COVID antibody programs. They will question whether durability against future variants can be maintained, whether regulators will support broad-population use, and whether consumers and physicians will adopt an antibody-based prevention strategy at scale. They will also scrutinize commercial execution, reimbursement, administration logistics, and future capital requirements. Even with positive clinical data, skeptics will argue that demonstrating efficacy is only the first step toward building a large and sustainable prevention market. If the program encounters delays, disappointing efficacy, regulatory setbacks, or adoption concerns, investor attention could quickly shift back toward PEMGARDA growth limitations and long-term financing risk.

Confidence in mechanism

Confidence in mechanism should remain heavily discounted despite management’s increasingly ambitious vision for antibody-based COVID prevention. The core mechanistic claim is that engineered monoclonal antibodies targeting highly conserved viral epitopes can provide durable, high-level protection against SARS-CoV-2 while remaining resilient to the viral evolution that rendered prior COVID antibodies obsolete. Conceptually, the thesis is attractive. In practice, virtually every major COVID antibody program initially appeared durable until viral evolution eventually proved otherwise.

The first structural issue is that the entire investment case depends on a prediction about future viral evolution. Management argues that its antibodies target regions of the spike protein that are less prone to mutation and therefore should retain activity across emerging variants. The problem is that SARS-CoV-2 has repeatedly demonstrated an ability to evolve around seemingly durable therapeutic approaches. Investors have watched multiple generations of monoclonal antibodies move from highly effective to functionally obsolete within relatively short periods. The burden of proof is therefore exceptionally high whenever a company claims it has finally solved the variant problem.

• Mechanistic attribution is also challenging because much of the supporting evidence comes from neutralization assays, epitope mapping, structural biology, and laboratory models rather than years of prospective clinical validation against future variants. High neutralizing activity in vitro does not necessarily guarantee durable protection in real-world populations. History suggests that viral evolution often exposes weaknesses that were not apparent during earlier laboratory characterization. Investors should be careful not to over-extrapolate laboratory durability into long-term commercial durability.

• The platform’s central assumption is that antibody engineering can consistently outpace viral mutation. While management presents VYD2311 as an evolution of earlier programs rather than a completely new discovery effort, that creates a different risk. The need for continuous antibody redesign may ultimately demonstrate adaptability, but it could also suggest the platform remains trapped in a perpetual race against viral evolution. If future variants require repeated optimization cycles, commercial scalability and regulatory simplicity may prove more difficult than currently assumed.

• Confidence should also be discounted because the mechanism remains largely unproven outside of narrow populations. PEMGARDA has demonstrated commercial viability in immunocompromised patients, but VYD2311 seeks to expand into a dramatically larger and healthier population. Mechanisms that work well in high-risk patients do not automatically translate into mass-market prevention products. The evidence required to justify widespread administration in healthy individuals is substantially higher from both efficacy and safety perspectives.

• The Long COVID narrative introduces additional mechanistic uncertainty. The company’s broader vision increasingly incorporates the hypothesis that persistent viral reservoirs or circulating spike antigen contribute to chronic symptoms and that prolonged antibody exposure may help address those underlying drivers. While biologically plausible, this remains an actively debated area of science without definitive clinical validation. Investors should avoid assigning meaningful value to these mechanistic extensions until controlled studies demonstrate reproducible clinical benefit.

• Repeat-dose administration creates another underappreciated risk. The company anticipates that some individuals may choose more frequent dosing schedules to maintain protection. While this may improve efficacy, it also increases the importance of long-term safety, immunogenicity, tolerability, and adherence. Mechanisms that appear straightforward in single-dose studies often encounter new challenges when administered repeatedly across large populations over multiple years.

• Commercial mechanism risk may ultimately be the most important risk of all. Even if the biological mechanism works exactly as intended, investors still need to determine whether antibody-based prevention becomes a behavior that consumers and physicians actually embrace. The company is effectively asking the market to adopt an entirely new category of prevention rather than improving an existing one. Scientific success alone does not guarantee behavioral adoption. The history of healthcare is filled with biologically effective products that failed because they could not overcome physician habits, patient preferences, reimbursement barriers, or logistical complexity.

• Most importantly, the mechanism remains largely forward-looking rather than fully validated. The central thesis is not that antibodies can neutralize COVID today. That has already been demonstrated repeatedly. The thesis is that this specific antibody architecture can maintain meaningful protection against future variants while supporting broad commercial adoption. Those are much larger claims that remain largely unproven.

What upgrades confidence: successful Phase 3 efficacy results from DECLARATION, demonstration of protection across evolving circulating variants, evidence that activity remains durable without frequent redesign, favorable safety data in broad populations, regulatory alignment supporting large-scale use, and real-world adoption that demonstrates consumers and physicians are willing to embrace antibody-based prevention as a genuine alternative to vaccination rather than a niche adjunct therapy.

PK, exposure, and distribution

PK and exposure provide some of the stronger evidence supporting the VYD2311 thesis, but they should not be confused with proof of commercial viability or long-term clinical success. The company’s central claim is that VYD2311 can generate sustained neutralizing antibody levels capable of providing durable protection against COVID while maintaining activity against evolving variants. Unlike many development-stage biotechnology stories where exposure itself is uncertain, the fundamental biology here is relatively straightforward: administer a monoclonal antibody, achieve measurable circulating antibody concentrations, and maintain neutralizing activity over time. The real debate is not whether exposure occurs, but whether that exposure remains clinically relevant as the virus continues to evolve.

To management’s credit, PK may be one of the least controversial parts of the story. Antibody therapies have decades of precedent demonstrating predictable systemic exposure, measurable half-lives, and dose-dependent increases in circulating concentrations. The company has also engineered half-life extension features specifically intended to prolong protection. Relative to many biotech programs where the mechanism itself remains speculative, VYD2311 benefits from operating within a modality that is already well understood.

That said, investors should be careful not to overstate what PK actually de-risks. High antibody concentrations are only valuable if the antibody continues to neutralize circulating variants. The history of COVID monoclonal antibodies is littered with programs that achieved excellent exposure profiles yet became commercially irrelevant when viral evolution reduced functional activity. In those cases, the problem was never drug exposure. The problem was that exposure no longer translated into protection. VYD2311 still faces that same fundamental risk.

• The relationship between exposure and efficacy also remains partially inferential. Much of the company’s confidence rests on maintaining neutralizing antibody titers above thresholds believed to be protective. While this framework is biologically reasonable and has precedent in infectious disease development, the exact correlation between antibody levels and real-world prevention remains imperfect. Ultimately, regulators and investors will care far more about observed reductions in symptomatic COVID than about antibody concentrations alone.

• Distribution introduces another layer of uncertainty. VYD2311 is being developed with ambitions that extend far beyond the immunocompromised population currently served by PEMGARDA. Achieving broad exposure across healthy individuals requires more than favorable PK. It requires convenient administration, physician acceptance, patient willingness to receive repeat dosing, reimbursement support, and scalable distribution infrastructure. Strong PK can support the clinical rationale, but it does not solve the adoption challenge.

• Repeat dosing may also create risks that are not fully visible today. The company has expressed interest in generating data that could support periodic redosing and potentially enhanced protection for individuals seeking additional coverage. While sustained exposure is generally a positive attribute, repeated administration increases the importance of long-term safety, immunogenicity, adherence, and tolerability. These questions often remain unresolved until substantially larger populations have been treated over longer periods.

• Perhaps most importantly, PK cannot answer the central commercial question. Even if VYD2311 consistently generates protective antibody concentrations for extended periods, investors still need evidence that consumers and physicians view antibody-based prevention as sufficiently differentiated from vaccination to justify adoption. Exposure can support efficacy. It cannot create demand.

What upgrades confidence: demonstration that antibody concentrations translate into meaningful reductions in symptomatic COVID in Phase 3 studies, evidence that protection persists against newly emerging variants, durable neutralization activity that remains correlated with observed efficacy, favorable repeat-dose safety data, and real-world outcomes confirming that sustained exposure produces clinically meaningful protection outside controlled trial settings.

PD read-through

PD read-through is directionally supportive, but far less de-risking than bulls often assume. The company’s central pharmacodynamic thesis is straightforward: maintain sufficiently high neutralizing antibody activity for long enough and COVID infections should decline. Unlike many therapeutic programs where the relationship between target engagement and clinical activity remains uncertain, neutralizing antibodies have a well-established biological rationale. The challenge is not proving that antibodies can neutralize SARS-CoV-2. The challenge is proving that neutralization remains durable and clinically meaningful as circulating variants continue to evolve.

• The strongest aspect of the PD story is that the mechanism itself has already been validated at a high level. COVID antibodies have repeatedly demonstrated the ability to prevent infection and reduce disease risk when neutralization activity remains intact. That historical precedent removes much of the target-engagement uncertainty that often exists in early-stage biotechnology programs. Investors do not need to believe a novel pathway is being discovered. They only need to believe the antibody can continue working against future variants.

• That distinction is important because most of the remaining PD uncertainty stems from durability rather than biological plausibility. Management frequently emphasizes neutralization potency, conserved epitopes, and sustained activity against contemporary variants. While encouraging, these measurements remain surrogate markers. The real pharmacodynamic endpoint is prevention of symptomatic COVID in the real world. Numerous prior antibodies demonstrated excellent neutralization metrics before viral evolution eventually reduced clinical utility. Investors should therefore be cautious about overinterpreting laboratory potency as proof of durable protection.

• The second challenge is that the exact relationship between neutralizing titers and long-term efficacy remains imperfectly understood. Higher titers generally correlate with greater protection, but the magnitude and duration of that protection can vary depending on the variant landscape, exposure intensity, host factors, and population characteristics. Strong PD signals today do not necessarily guarantee equivalent performance against future viral lineages.

• Population expansion introduces another source of uncertainty. PEMGARDA’s use in immunocompromised patients provides evidence that antibody-mediated prevention can be clinically useful, but VYD2311 seeks to expand into much broader populations. The PD requirements for widespread preventive use are substantially more demanding. Small reductions in efficacy that may be acceptable in high-risk populations become much more important when targeting healthy individuals who already have alternative prevention options.

• The Long COVID opportunity introduces additional pharmacodynamic complexity. The hypothesis that prolonged neutralization could reduce persistent viral reservoirs or circulating spike antigen remains biologically plausible, but it is far from proven. Investors should view any pharmacodynamic assumptions surrounding Long COVID as exploratory rather than validated. The mechanism may ultimately work, but there is currently insufficient evidence to conclude that reductions in viral markers will reliably translate into meaningful clinical improvement.

• Perhaps the biggest PD risk is that the company is attempting to predict future biology rather than measure current biology. Existing data can demonstrate activity against today’s variants. What investors ultimately need is confidence that those pharmacodynamic relationships persist as the virus evolves over the coming years. That is a much more difficult claim to validate prospectively.

• Overall, the data helps de-risk the mechanism at the biological plausibility level. Neutralizing antibodies clearly work when they retain activity. What remains unproven is whether this specific antibody architecture can sustain meaningful pharmacodynamic protection against future variants while supporting broad adoption outside specialized patient populations. The upcoming efficacy studies are therefore far more important than any individual neutralization assay or laboratory potency metric.

What upgrades confidence: strong symptomatic COVID prevention data from DECLARATION, preservation of efficacy despite variant turnover, consistent correlation between neutralization activity and observed clinical outcomes, evidence supporting repeat-dose durability, and real-world effectiveness data demonstrating that pharmacodynamic activity translates into sustained protection outside controlled clinical settings.

Clinical Trial Design

Confidence in the VYD2311 clinical program should be measured, not assumed. The REVOLUTION program is designed to answer the right regulatory question: can VYD2311 prevent symptomatic COVID with an acceptable safety profile? That is the correct question for approval, but it is not the same as proving broad commercial adoption, durable variant coverage, or mass-market demand for antibody-based prevention.

• The key strength of the design is that DECLARATION is a randomized, placebo-controlled Phase 3 efficacy study. That matters. This is not just a neutralization assay story or an immunobridging exercise. The program is attempting to generate prospective clinical evidence that VYD2311 reduces symptomatic COVID. If successful, that would meaningfully de-risk the biology and give investors a cleaner regulatory path than a purely surrogate-driven package.

• The first design risk is event dependence. COVID incidence is unpredictable, seasonal, geographically variable, and influenced by prior immunity, vaccination behavior, testing behavior, and variant circulation. A prevention study can be well designed and still struggle if attack rates are lower than expected or if symptomatic cases do not accumulate cleanly. This creates timing risk and statistical risk that are partly outside the company’s control.

• The second risk is variant timing. The study can only test efficacy against the variants circulating during the trial. A positive result would support the mechanism, but it would not permanently solve the durability question. If the variant landscape shifts materially before review, launch, or commercialization, investors may still have to haircut the data. That is the core structural problem with COVID antibodies.

• The third risk is population generalizability. Looking at the analysts results and price coverage, those who cover the company rate as a STRONG BUY with 3/3 buy ratings. Keep in mind analyst ratings ALWAYS must be taken with a grain of salt as bearish analysts do not have long careers for obvious reasons.

The stock is currently trading at $1.03. The lowest analyst estimate has this one trading at $10.00. The upside scenario is at $10 - ~10x upside with downside potential over the longer timeframe.

If we look at options pricing, and specifically IMPLIED VOLATILITY we can get a sense of the size of the move the traders are projecting. IF you do the math you get about a ~+115% move to the upside, and a -90% move to the downside. That would put the options projected price projections at ~$2.50 to the upside and ~$0.10 to the downside. Again, this is for a play on the short term catalyst which is a different timeframe from the 1yr analyst targets shown above.

Introduction: When Cholesterol Isn't the Only Culprit

For decades, the focus in cardiometabolic disease has revolved around LDL cholesterol. Statins, PCSK9 inhibitors, and ezetimibe reshaped the landscape. But beneath the surface lies another class of particles (triglyceride-rich lipoproteins and their remnants) that drive inflammation, insulin resistance, and organ damage in their own right. These are not just silent passengers in the bloodstream. They’re active saboteurs.

These aren't your classic statins or PCSK9 inhibitors. They don’t reduce production or absorption. They restore clearance. It's biology that’s cleaner, more targeted, and tuned to the root dysfunction. The clearest signals so far are in hypertriglyceridemia, pancreatitis, and fatty liver, but the underlying theme is broader: remnant cholesterol, insulin resistance, and tissue inflammation all start upstream with poor lipid partitioning.

What’s emerging now is a therapeutic framework that targets the regulatory brakes on lipoprotein lipase (LPL), the enzyme responsible for clearing these dangerous particles. Instead of simply blocking production or absorption, these new agents restore the body’s ability to hydrolyze, clear, and rewire lipid trafficking. And in doing so, they offer new hope across three pathologies that rarely intersect in the same treatment playbook: acute pancreatitis, atherosclerosis, and steatohepatitis.

The therapies in question (ApoCIII inhibitors, ANGPTL3/4/8 inhibitors, and FGF-21 analogue) share a common thread: they each increase clearance of triglycerides, either directly through LPL or indirectly by modulating hepatic lipid metabolism.

Together, they represent a new axis in cardiometabolic disease. The question is not whether they work, but where, in whom, and how soon we’ll see outcomes that justify adoption.

Lipoprotein Lipase: The Forgotten Switch

LPL sits at a metabolic crossroads. It hydrolyzes triglycerides from circulating lipoproteins, releasing free fatty acids for uptake by muscle, adipose, and other tissues. But its activity is tightly regulated (by proteins like ApoCIII, ANGPTL3, ANGPTL4, and ANGPTL8) which act like dimmer switches, dialing lipolysis up or down depending on nutritional state and tissue context.

The therapeutic logic is simple: inhibit these brakes, and LPL activity surges. That reduces plasma triglycerides, shrinks remnants, and potentially lowers inflammatory burden in the vasculature and pancreas. But biology rarely rewards simplicity. Each regulator has distinct expression patterns, tissue selectivity, and off-target liabilities. That’s where the recent wave of precision therapeutics enters.

Today, we’re seeing the emergence of RNA-targeted agents, antibodies, and analogues that precisely modulate these pathways.

ApoCIII inhibitors like olezarsen are already approved for severe hypertriglyceridemia. ANGPTL3 inhibitors like evinacumab have shown promise in rare lipid disorders. And FGF-21 analogues like efruxifermin offer a metabolic counterpunch for the liver.

These aren’t statin competitors, they’re orthogonal tools for a different class of lipid-driven disease.

Acute Pancreatitis: A Crisis of Triglyceride Overflow

When triglycerides cross the 1,000 mg/dL threshold, the risk of acute pancreatitis spikes.

Chylomicron particles clog capillaries in the pancreas, releasing toxic free fatty acids that trigger inflammation and necrosis. It’s a high-stakes, high-cost event, often requiring ICU-level care, and for decades, we’ve lacked targeted tools to prevent it.

That’s changing.

ApoCIII inhibitors have already proven effective at reducing triglycerides by 70-80% in patients with familial chylomicronemia syndrome (FCS) and severe hypertriglyceridemia. More importantly, they’ve been linked to real-world reductions in pancreatitis episodes. This is a clinical and economic win.

ANGPTL3/8 inhibition may offer a complementary path by acting on both fasting and postprandial lipids.

These regulators influence lipase activity at different timescales and tissues.

A well-calibrated combo approach could address daily fluctuations in triglyceride handling, preventing spikes that trigger pancreatitis in vulnerable patients.

Expect this use case to expand, particularly in populations that traditional fibrates and fish oils failed to protect.

Atherosclerosis: Beyond LDL, the Remnant Revolution

LDL cholesterol may be the kingpin of atherosclerosis, but remnant particles, those leftover from triglyceride-rich VLDL and chylomicrons, are gaining notoriety as co-conspirators. They’re rich in cholesterol, pro-inflammatory, and sticky to the endothelium. And unlike LDL, they’re not always cleared by statins or PCSK9 inhibitors.

That’s where LPL-activating strategies shine.

By accelerating remnant clearance, these drugs reduce atherogenic burden at a different point in the lipid cascade. Epidemiological data suggest that lifelong suppression of ApoCIII or ANGPTL3 correlates with lower cardiovascular risk, even in those with normal LDL levels.

But this is still a bet.

No major cardiovascular outcomes trial has yet proven that these agents reduce heart attacks or strokes.

The challenge is complex: identify the right patients (remnant-driven risk), design long enough trials, and prove additive benefit on top of existing lipid-lowering therapy.

If successful, we could be looking at a new class of ASCVD preventives, not for everyone, but for the millions with residual risk that LDL alone can’t explain.

MASH: The Liver as Lipid Sensor and Signal Hub

In the context of steatohepatitis, the problem isn’t just excess fat, it’s the inflammatory and fibrotic response it triggers.

FGF-21 analogues represent a novel attempt to rewire hepatic metabolism, not by directly targeting lipids, but by restoring insulin sensitivity and mitochondrial function in the liver.

Unlike LPL modulators, which act systemically and acutely, FGF-21 agents work more like chronic metabolic remodelers. They reduce de novo lipogenesis, improve β-oxidation, and downregulate inflammatory signaling. Clinical trials show clear reductions in hepatic fat content, and several agents have demonstrated fibrosis regression, something that eluded earlier NASH hopefuls.

The irony is that these agents, while metabolically active, don’t always lower plasma triglycerides as dramatically. Their success underscores a broader truth: not all triglyceride-driven diseases are created equal. In the liver, it’s not the plasma particle that matters most, but how the hepatocyte handles its internal load. FGF-21 may be less about lipase and more about load management.

ASOs: Antisense Meets Manufacturing Leverage

Modern ASOs are precise, liver-targeted, and commercially viable.

• GalNAc conjugation ensures hepatocyte delivery.

• Monthly dosing ensures compliance.

• The regulatory path is getting smoother with every approval.

But what’s coming next is even more interesting. We’re about to see whether ASOs can make the leap from rare disease into garden-variety hypertriglyceridemia.

That’s the question behind the CORE and CORE2 readouts, two pivotal Phase 3 trials run by Ionis, evaluating olezarsen, a GalNAc-conjugated antisense inhibitor of apolipoprotein C-III (ApoCIII). The drug is already approved as Tryngolza for familial chylomicronemia syndrome (FCS), a rare genetic disorder marked by extreme triglyceride elevations and recurrent pancreatitis. But these studies target a much broader population: patients with severe hypertriglyceridemia (≥500 mg/dL) due to more common metabolic dysfunction. If the results are positive, ApoCIII inhibition will move from orphan indication to frontline lipid therapy.

Behind olezarsen, Ionis and AstraZeneca are building modular ASO platforms where the backbone stays the same, chemistry, conjugation, delivery and only the RNA sequence changes. That allows them to rapidly pivot from ApoCIII to ANGPTL3, ANGPTL4, or future targets with the same CMC infrastructure. It’s a plug-and-play model that shrinks development timelines, lowers manufacturing risk, and gives them broad optionality across hepatic diseases.

This isn’t just about one drug. It’s about validating a format that can repeatedly deliver liver-targeted metabolic therapies with minimal reinvention—an approach that could redefine how lipid diseases are treated, trialed, and commercialized.

Most investors miss this: ASOs aren’t just drugs, they’re platforms. You don’t need to reinvent the wheel each time.

That opens the door to broad liver portfolios with lower marginal cost, faster development time, and the ability to adapt pipelines based on shifting biology or payer preferences.

Gene Editing: One Shot, One Future

Lilly’s $1B acquisition of Verve wasn’t a fluke—it was a flag in the ground. Editing is here, and it’s serious. VERVE-102, targeting PCSK9, is showing durable ~55% LDL-C reductions from a single dose. VERVE-201 aims at ANGPTL3 to lower remnants and Lp(a) simultaneously.

These aren’t incremental drugs. They’re replacements for chronic therapy.

Editing has a steep bar to clear (safety, off-target risk, delivery) but the upside is transformational. What statins do over 20 years, an editor could do in 20 minutes. And as cardiac-specific delivery matures, the potential shifts from blood lipid modulation to direct plaque remodeling, macrophage reprogramming, and vascular rejuvenation.

Don’t be surprised if the first muscle- or heart-directed CRISPR base editor hits the clinic before 2027. Academic groups are already working on this. The long game? Edit once, treat forever, and eliminate risk at its root.

Immune-Lipid Convergence: The Athero-Inflammatory Link

CANTOS taught us that inflammation matters. Blocking IL-1β reduced cardiovascular events independent of LDL-C. More recent work shows that immune checkpoints like PD-1 and CTLA-4 are active inside plaques, meaning we can modulate the immune tone of atherosclerosis, not just lipid content.

This opens up fascinating combinations:

• PCSK9 editing plus low-dose IL-6 blockade to address residual inflammatory risk.

• ApoCIII inhibition plus CD47-based macrophage reprogramming to clear necrotic cores.

These combos won’t show up in LDL curves, they’ll show up in plaque stability, efferocytosis, and long-term event rates.

If trials shift endpoints toward imaging and systems biology, we could see next-gen cardiometabolic regimens that blend lipid control and immune modulation in the same way oncology blends cytotoxic and checkpoint strategies.

Beyond the Liver: Delivery’s New Frontier

All of this depends on delivery. Right now, everything hits the liver. But LPL biology lives elsewhere, skeletal muscle, adipose, even the heart.

Two technologies are quietly shifting the map:

• Endosomal-escape peptides are enabling skeletal muscle targeting in primates.

• Self-assembling RNA-peptide condensates are steering payloads to brown fat with 10x less liver uptake.

These aren’t big pharma plays yet, they’re academic or stealth biotech bets. But they could open the door to editing in tissues that have long been out of reach.

Once that unlock happens, editing isn’t just a liver story, it’s a whole-body metabolic rewiring strategy. And the companies who see that early will be the ones building the next dominant platforms.

Wildcards: What Everyone Else Misses

Several themes are hiding in plain sight.

• ANGPTL4 inhibition may have crossover benefits in oncology, particularly in lipid-rich tumors where immune resistance is high.

• Circular RNA platforms could deliver LPL or ANGPTL3 protein every six months with a single injection, undercutting both ASOs and mRNA.

• And AI-driven “digital twin” lipid models could individualize edit sites and dosing schedules based on real-time lipidomic and immune data.

None of these are priced in. All of them are being quietly developed. And they’ll likely define the next leap in metabolic therapy, not with new targets, but with new ways to match target, tissue, and patient.

Translation Blueprint: What Comes Next

For fun we thought we would project how this all stacks up over time:

Short-term (2025–2027):
ASO expansion, inclisiran CV outcomes, first extrahepatic LNP trials.

Mid-term (2027–2029):
Gene editing labels in FH, combo trials with anti-IL-6 or anti-inflammatory agents, oral PCSK9 approvals.

Long-term (2030+):
Multi-tissue editing, circRNA boosters, immune-lipid hybrids, bundled pay-for-performance models tied to inflammation and event reduction.

The future is looking bright!

What the Trials Don’t Tell You Yet

New technology is always fun to talk about, but lets shift to where it must be proven out - in the clinic. So far, most of the excitement has been biochemical - large reductions in triglycerides, cleaner remnant profiles, and early signs of reduced organ stress.

But the leap from lipids to lives saved is nontrivial.

Most of these programs remain in Phase 2, and only a few have crossed the regulatory finish line in niche indications.

We still don’t know who benefits most:

• Is it the patient with baseline triglycerides of 300?

• Or the one with genetic defects in LPL regulation?

• What’s the added value if you’re already on a statin and GLP-1?

These questions aren’t just academic, they determine payer uptake, trial design, and ultimately, whether these drugs become niche tools or foundational therapies.

The next wave of trials must stratify patients not just by lipids, but by end-organ vulnerability.

A pancreatitis patient has different needs than a MASH patient, and cardiovascular risk reduction is its own beast entirely. Smart design will mean layering biomarkers, imaging, and functional outcomes, not just relying on triglyceride drop as the north star.

Strategic Implications for Biotech and Pharma

This shift toward LPL-enhancing therapeutics opens new strategic lanes for drug developers.

Companies that once competed in silos (liver disease, lipidology, endocrinology) now find themselves circling the same biology. That convergence creates both M&A opportunity and internal portfolio conflicts.

For platform companies, the challenge is proving that these are not one-trick assets. Can a single molecule straddle multiple indications and geographies? Can it scale from rare disease to broad cardiometabolic populations without losing efficacy or payer support?

For pharma, the question is timing. Outcomes trials take years, but if you wait for those to read out, the best assets may be gone.

We expect the most aggressive acquirers to be those with large metabolic franchises and unmet pipeline gaps, Novo Nordisk, Amgen, Lilly, and possibly Pfizer. Those with both GLP-1 and NASH ambitions have the most to gain from a diversified lipid play. It’s a question of complement, not cannibalization.

Why This Matters Now

The field isn’t about picking a single horse anymore. It’s about stacking optionality (ASOs, editing, immune modulators, targeted delivery) and building adaptive platforms that move with biology, not against it.

The winners won’t be those who lower lipids the most. They’ll be those who understand lipid logic, the networks, the nodes, and the new ways to intervene.

The urgency here isn’t theoretical.

• Acute pancreatitis admissions are rising.

• Cardiovascular risk remains stubbornly high despite LDL control.

• MASH is now the leading cause of liver transplantation in the U.S.

The common thread? Lipid mismanagement, not just cholesterol, but the entire cascade of remnant, triglyceride, and hepatic lipid flux.

We’re entering a new phase in metabolic medicine, one that requires a different lens. This isn’t about dieting patients into lower LDL or running ever-larger statin trials. It’s about targeting lipid handling at the source, via enzymes like LPL and hormonal networks like FGF-21. The tools exist. The biology is clear. What remains is the will to invest in smarter trial design and longer-term thinking.

It’s no coincidence that multiple biotech firms are converging here at once. The same way PCSK9 changed the LDL story, this class could reshape the next generation of cardiovascular and metabolic disease therapeutics, if the data keep pace.

CONCLUSION

Today, investors and operators alike face a clear decision point. The science behind triglyceride-lowering and LPL-enhancing drugs is mature, the mechanisms are validated, and regulatory traction is building. What remains is clarity on where the biggest wins will be (pancreas, heart, or liver) and which programs will generate the outcomes data to unlock broader adoption. Lilly and Novo face off at ADA 2026 as others seek to compete in obesity

At the 2026 American Diabetes Association Scientific Sessions, the obesity drug sector will take center stage amid soaring sales projections, with IQVIA forecasting up to $200 billion by 2027. Market leaders Eli Lilly and Novo Nordisk are set to present new data on next-generation obesity drugs. Lilly’s triple-G agonist retatrutide continues to show remarkable efficacy, producing up to 28.3% weight loss over 80 weeks in recent Phase 3 trials, though safety concerns – particularly dysesthesia – remain. Novo Nordisk will deliver additional results for CagriSema, a fixed-dose combination of semaglutide and cagrilintide, which demonstrated slightly less weight loss than Lilly’s Zepbound in direct comparison, but Novo anticipates it will launch in the US in 2027 with a strong initial label as the first amylin-based product. Both companies’ presentations are anticipated to inform ongoing clinical debate around efficacy, tolerability, and new drug categories emerging within obesity treatment.

Competitors are racing to secure third place in the market. Boehringer Ingelheim, via a license from Zealand Pharma, will present Phase 3 data on survodutide, which achieved a 16.6% weight reduction and is positioned as a dual agonist targeting broader metabolic dysfunction and liver disease, though with weight-loss efficacy currently trailing the leaders. Roche, following its $2.7 billion acquisition of Carmot Therapeutics, will present additional data for its GLP-1/GIP agonist CT-388, which showed a 22.5% placebo-adjusted weight reduction at 48 weeks, and is exploring combination regimens. Amgen remains in the hunt with MariTide, displaying up to 20% body weight reduction in Phase 2 but is currently not presenting new data. Smaller biotechs such as Structure Therapeutics and Viking Therapeutics will also present competitive oral and injectable obesity candidates, with both companies reporting double-digit placebo-adjusted weight reductions and advancing quickly into later-stage trials, signaling a rapidly diversifying and intensifying obesity drug development landscape.

NewLimit snags $435M after seeing age reversal in human liver cells

NewLimit, a California-based biotech focused on anti-aging therapies, has raised $435 million in a Series C round led by Founders Fund with participation from new and returning investors including Eli Lilly Ventures, Thrive Capital, Greenoaks, Quiet Capital, Kleiner Perkins, Nat Friedman, and Daniel Gross. This financing follows a previous $130 million raise and is intended to advance NewLimit’s first epigenetic reprogramming medicine into clinical trials, now expected to enter human testing next year—a significant acceleration from the originally forecasted decade-plus timeline. The company’s approach centers on reversing cellular aging, highlighted by a reported breakthrough where a prototype therapy reversed cell age in old human liver cells, supporting the concept that cellular aging is reversible and potentially treatable.

NewLimit, founded in 2021 by Coinbase CEO Brian Armstrong, bioengineer Blake Byers, and computational biologist and current CEO Jacob Kimmel, is developing artificial intelligence and genomics technologies across programs in metabolic, vascular, and immune health, and has stated intentions to expand its therapeutic pipeline. The lead program aims to enhance liver recovery after injury, mitigate damage from dietary stress, and accelerate post-alcohol recovery. The company positions itself at the forefront of the burgeoning anti-aging field—attracting significant tech investor interest—alongside recent high-profile AI-driven biotech fundraises such as Earendil Labs and Isomorphic Labs, but differs by publicly sharing more details of its anti-aging research progress.

ADC Therapeutics nosedives on 27 deaths in confirmatory trial for lymphoma drug

ADC Therapeutics’ share price fell over 50% following disclosure that 27 patients died in the Zynlonta arm compared to 9 in the control group during the confirmatory Phase 3 LOTIS-5 trial for relapsed or refractory diffuse large B cell lymphoma (DLBCL). Zynlonta, an anti-CD19 antibody-drug conjugate that received FDA accelerated approval in 2021 for third-line or later DLBCL, was being evaluated for potential use in earlier treatment lines. The trial, which enrolled 440 patients, compared Zynlonta plus rituximab against rituximab with gemcitabine and oxaliplatin. The deaths, reported as treatment-emergent adverse events, were not definitively linked to Zynlonta, but the company acknowledged a “majority” occurred in patients aged 75 or older and attributed the higher rates partially to longer observation in the experimental arm.

Despite meeting its primary endpoint by showing a significant progression-free survival benefit (27% risk reduction of death or progression), Zynlonta did not demonstrate an overall survival advantage compared to the control regimen. Both overall and complete response rates were higher in the Zynlonta arm, but statistical significance was not disclosed. The safety concerns and lack of survival benefit have raised doubts among analysts regarding Zynlonta’s potential use in second-line settings and could complicate its path to full regulatory approval. ADC Therapeutics intends to present the LOTIS-5 data to the FDA and submit a supplemental approval application for Zynlonta plus rituximab in the fourth quarter.

ASCO’s biggest winners showcased a new era of cancer innovation

At the 2024 American Society of Clinical Oncology (ASCO) meeting, Revolution Medicines emerged as a key clinical and commercial winner, driven by detailed Phase 3 RASolute 302 trial data showing its RAS inhibitor daraxonrasib nearly doubled both overall survival and progression-free survival, and nearly tripled objective response rates in pancreatic cancer compared to chemotherapy. The funds will support the advancement of the company’s lead asset, zolucatetide, derived from the Helicon peptide platform, through ongoing dose-expansion and into Phase 3 trials for desmoid tumors, as well as development in other indications such as familial adenomatous polyposis and hepatocellular carcinoma. Parabilis intends to list on the Nasdaq under the ticker PBLS, joining a recent wave of sizable biotech IPOs, indicating renewed investor enthusiasm in the sector.

Strategically, Parabilis recently secured a collaboration with Regeneron Pharmaceuticals, including a $75 million investment and a partnership to develop antibody-helicon conjugates (AHCs), with the deal’s total milestone and royalty potential reportedly reaching up to $2.3 billion. Legend surges on early data for ‘in vivo’ lymphoma cell therapy

The bill, prompted by major deals such as Bristol Myers Squibb’s alliance with Hengrui Pharma (potentially worth $15.2 billion) and Pfizer’s partnership with Innovent Biologics (over $10 billion), seeks to restrict licensing deals, joint ventures, and equity investments in China involving sensitive technology and intellectual property, with oversight from both the Treasury and Defense Departments. BIOTECH NEWS UPDATES FOR THE WEEK 5/25

Pfizer bets up to $10.5B in 12-candidate cancer collab with China’s Innovent

Pfizer has entered into a strategic collaboration with China’s Innovent Biologics valued at up to $10.5 billion, focusing on developing a pipeline of 12 early-stage antibody-based oncology therapies, including antibody-drug conjugates (ADCs) and multi-specific antibodies. The agreement includes a $650 million upfront payment from Pfizer and up to $9.85 billion in development, regulatory, and commercial milestone payments, with Innovent contributing eight assets—four of which Pfizer will have exclusive global rights to, while the other four will be licensed outside Greater China. Pfizer is the primary funder for the Innovent-contributed programs, and will also add four of its own assets to the collaboration. The companies plan to co-develop and co-commercialize these assets, sharing development costs and splitting profits in the U.S. and Europe, while Innovent retains rights in Greater China. The transaction is expected to close in the third quarter of 2024.

Focus will center on absolute ORR, depth and durability of response, and whether PFS trends remain intact as follow-up matures. The study includes crossover to lifileucel after progression, which may complicate eventual survival interpretation and create debate around magnitude of benefit versus timing effects. Operational execution also matters because lifileucel remains a complex individualized cell therapy requiring manufacturing, lymphodepletion, and IL-2 support. Current turnaround is roughly 32 days from enrollment through treatment, which risks becoming a commercial constraint if scaling into larger frontline populations proves difficult.

The broader risk is that expectations already extend well beyond refractory melanoma. The platform narrative increasingly depends on proving TIL therapy can scale across frontline melanoma, NSCLC, and additional solid tumors. If randomized data looks directionally positive but operationally cumbersome or only modestly differentiated from existing checkpoint standards, the story can quickly shift from “foundational solid tumor platform” to a high-complexity niche cell therapy with limited real-world adoption.

Options are already pricing a highly binary event, with the current straddle matrix implying roughly ±40-70% moves across the core strike range and >100% upside scenarios at higher strikes into expiration. The market is clearly positioned for a decisive outcome rather than a modest beat-or-raise setup. Breakevens remain wide and require an immediate rerating in either commercial expectations or platform credibility to pay out meaningfully. IV should collapse sharply post-readout, so outcomes that are statistically positive but operationally messy, commercially ambiguous, or reliant on subgroup framing risk rapid premium decay and failed volatility capture despite superficially “good” data.

HOW TO PLAY BIOTECH APPROVAL/DATA READOUTS ANNOUNCEMENTS

While biotech equities will move on news of announcement, often if there is high conviction in a positive read, a large run up ahead of the news will occur and the actual event turns into a sell the news event. Sometimes, investors wait for the actual event to make a long/short decision. And obviously if the news is disappointing (more often than not) the stock will tank.
