BIOTECH MARKET RESEARCH $CADL | Ep. 1029

Candel Therapeutics ($CADL) is a catalyst-driven setup centered on the expected Q4 2026 BLA submission for aglatimagene besadenovec in newly diagnosed intermediate- to high-risk localized prostate cancer. This is not a platform catalyst. It is a very specific regulatory test: can a 745-patient randomized Phase 3 trial with a positive disease-free survival endpoint support approval for an intraprostatic viral immunotherapy added to standard radiation with or without short-course ADT. The bull case is that the trial was randomized, placebo-controlled, conducted under SPA, and hit DFS in the ITT population with HR 0.70, p=0.0155. The cleaner prostate cancer-specific DFS cut also favors treatment, with HR 0.62, p=0.0046, and extended follow-up showed HR 0.61, p=0.0031 at 58 months median follow-up.

The bear case is that the approval argument still leans on recurrence-based endpoints, not survival. Overall survival was similar at the earlier analysis, with only 2 prostate cancer deaths total, one in each arm. The downstream endpoints look directionally supportive but not definitive: time to new anticancer therapy HR 0.72, biochemical failure HR 0.72, and time to metastasis HR 0.58 all have confidence intervals crossing 1.0. The intermediate-risk metastasis cut looks optically strong at 1/422 vs 5/213, HR 0.10, but that is a tiny event-count subgroup. Bulls will call this consistency across endpoints. Bears will call it a positive primary endpoint surrounded by low-event, clinically immature secondary evidence.

Safety is probably not the main pushback. Aglatimagene looks tolerable in the prostate setting, with mostly grade 1-2 treatment-related AEs, grade 3 TRAEs under 5%, no grade 4+ TRAEs, and discontinuations due to AEs that were actually slightly lower than placebo, 5.4% vs 6.0%. The bigger issue is commercial and practical. The product requires 3 intraprostatic injections, 14 days of valacyclovir after each course, and coordination around radiation. That may be acceptable if guidelines and reimbursement are clean. It becomes a lot harder if FDA gives a narrow label, NCCN support is weaker than expected, or physicians view the benefit as too incremental for a localized prostate cancer population where many patients already do well for years.

So the catalyst is not just “BLA submitted.” The catalyst is whether the filing frames aglatimagene as an approvable, commercially credible add-on to curative-intent radiation, or exposes the gap between statistical DFS benefit and real-world adoption. Positive BLA progress would let investors underwrite CADL as a near-commercial oncology story with a differentiated localized prostate cancer asset. Regulatory hesitation would shift the stock back toward cash, optionality, and a harsher read on the platform, especially because the company’s runway into Q1 2028 still leaves little room for a long regulatory detour before financing risk comes back into focus.

Every Monday we are out with public biotech research, profiling biotech companies with near term catalysts. This will focus primarily on the major market moving biotech events such as data readouts from the major scientific conferences as well as potential regulatory approvals.

We will not offer specific trading advice (and we do not hold any publicly traded biotech equity), but rather flag emerging events that will likely materially drive stock prices (sometimes +/-100% in either direction).

HOW TO PLAY BIOTECH APPROVAL/DATA READOUTS ANNOUNCEMENTS

While biotech equities will move on news of announcement, often if there is high conviction in a positive read, a large run up ahead of the news will occur and the actual event turns into a sell the news event. Sometimes, investors wait for the actual event to make a long/short decision. And obviously if the news is disappointing (more often than not) the stock will tank.

This is not financial advice so you will have to make your own call on how to best play these events. Our aim is simply to flag catalysts with high projected volatility.
