$RARE (Ultragenyx Pharmaceutical) is a regulatory-catalyst setup centered on the September 19, 2026 PDUFA action date for UX111, the company’s AAV9 gene therapy for Sanfilippo syndrome type A (MPS IIIA). MPS IIIA is a fatal lysosomal storage disease with early childhood onset, rapid neurodegeneration, and no approved therapies. Ultragenyx estimates roughly 3,000-5,000 patients in commercially accessible geographies.
The core regulatory question is whether the biomarker and developmental package is sufficient to support approval. More than 80% of patients in the efficacy set achieved at least a 50% reduction in CSF heparan sulfate, with median reductions of 63.98% in the efficacy set and 62.58% in the mITT set. The mITT group also showed a +23.2-point difference in Bayley-III cognitive raw score versus natural history (p<0.0001), with statistically significant differences in expressive communication, receptive communication, and fine motor. Gross motor numerically favored UX111 but did not reach significance.
The main bear argument is that the developmental comparisons rely on natural history rather than a randomized concurrent control, leaving FDA room to question comparability and how much clinical benefit can be attributed to treatment. Safety has been described as generally manageable, but AAV-related immune risk, manufacturing execution, and inspection risk remain relevant to the decision.
The market is pricing substantial event risk. At a share price of $26.62, the October 16, 2026 $27.50 straddle costs $15.35, implying a total straddle spectrum of approximately $11.27 to $41.97, or about -57.7% to +57.7% from spot. This is the full expected move priced by the straddle, not a probability-weighted forecast of where the stock will trade.
An approval with a commercially workable label could support the upside case, while a CRL, delayed action, manufacturing issue, safety restriction, or narrow label could pressure the shares. The most defensible framing is that UX111 is a high-variance regulatory binary where the key debate is not whether the therapy shows biological activity, but whether FDA considers the biomarker and natural-history-supported clinical evidence sufficient for approval.
Every Monday we are out with public biotech research, profiling biotech companies with near term catalysts. This will focus primarily on the major market moving biotech events such as data readouts from the major scientific conferences as well as potential regulatory approvals.
We will not offer specific trading advice (and we do not hold any publicly traded biotech equity), but rather flag emerging events that will likely materially drive stock prices (sometimes +/-100% in either direction).
Be sure to SUBSCRIBE to not miss.
HOW TO PLAY BIOTECH APPROVAL/DATA READOUTS ANNOUNCEMENTS
While biotech equities will move on news of announcement, often if there is high conviction in a positive read, a large run up ahead of the news will occur and the actual event turns into a sell the news event. Sometimes, investors wait for the actual event to make a long/short decision. And obviously if the news is disappointing (more often than not) the stock will tank.
This is not financial advice so you will have to make your own call on how to best play these events. Our aim is simply to flag catalysts with high projected volatility.
