This week we’re breaking down one of the most common and most misleading narratives in biotech right now: the gap between headline patient populations and actual revenue opportunity. Companies love to highlight “millions of patients” and massive unmet need, but that top-line number is only the starting point. We walk through the real funnel, from diagnosis and eligibility to access, reimbursement, and provider capacity, and show how quickly that population shrinks at each step. The result is a much smaller, more constrained market than most investors assume, where logistics, payer behavior, and system throughput matter just as much as clinical efficacy. The core question isn’t how many patients exist, it’s how many actually make it through the system.
